Make sure that the Header and Footer information has been has been turned off
on your Page Setup before printing these pages.

* All players attending a Scenario or Big Game at Wayne's World
MUST Read the Rules of Play and sign the
Letter of Understanding
along with the forms below.

ADULTS:
Please print and copy as many of these forms as you need
and have one of each filled out by each adult (Over 18 years of age) player.

MINORS:
Please print and copy as many of these forms as you need
and have one of each completed for each minor player (10 to 18 years of age)
and be sure each one is signed by a parent or guardian.
